Success Defined: A Student-Centered Transition Planning Project

The Center for Excellence in Developmental Disabilities at the UC Davis MIND Institute held a day-long training for transition-age youth and their parents on October 9.

Success Definedis an innovative project designed to empower students and parents to identify the elements of a successful life and to establish the action steps needed to achieve one. Families learned to customize plans and supports that meet the preferences, goals, and ambitions of their student using evidence-based materials and proven practices. Development of a family-driven vision statement and student-centered mission statement laid the foundation for a path to success. Facilitated work teams, coaching calls, community mentors, online networking, resource development, and web-based education are offered to participating families throughout the current school year.
