This consumer-friendly workbook was designed to assist people with disabilities in making decisions about the end of their life. It is available in PDF and hard-copy form in English, Spanish, and Chinese.  A DVD is also available. 

This workbook and DVD are a project of the California Department of Developmental Services (DDS) and was funded by a 2006-07 DDS Wellness grant.
