OHSU UCEDD Hosted its 4th Annual Transition Fair

The OHSU UCEDD hosted their 4th annual Transition Fair: Passport to a Healthy Future on May 20th, 2017 at Glencoe High School in Hillsboro. Families and providers from the area came out to learn more about community resources to help navigate the transition from pediatric to adult services.

The Transition Fair began 4 years ago with the goal of reaching families and individuals experiencing disability who are working toward a more seamless transition into adult services. Attendance of the fair has grown with each year, now hosting more than 50 participants with 24 vendors from the Portland metro area.

This year the fair was brought to the community of Washington County and a panel of family members and self-advocates shared their experience with navigating transition. Attendees got to visit with the more than 20 vendors and resource navigators to help take advantage of the resources available to them in the community.

The Transition Committee is looking forward to expanding this event to more communities in the area and helping more individuals and families with the task of transition.
