Although the new NIRS database is an online data entry system, AUCD recognizes that many individuals across the network work in the field and away from their program. Therefore, with the help of several programs AUCD has created sample standard paper forms for each dataset, to be used by program staff to collect data while in the field.

The forms below may be used directly to capture information while people are working away from their programs, or as templates to create a form that is more suitable for your program.

*Please note that that if your program develops its own unique form, it is important to compare the form created at your program to the templates, as the templates contain crucial information (such as check all that apply vs. please choose one).
