Attendees from this meeting included LEND, LEAH, and PPC program directors and staff. The purpose of this meeting was to form a Transition Work Group, and develop an action plan for joint transition projects over the next 1-2 years.  The individuals involved in this work group are willing to commit to ongoing work on a 1+ year basis with regular conference calls and meetings.
