EXPERIENCE 2001 - Present Wyoming INstitute for Disabilities, University of Wyoming Laramie, WYTitle: Project Coordinator, Senior; Core Indicators and Satisfaction Surveys. ICAP Assessments. Duties: Coordinate National Core Indicators data collection throughout Wyoming including 400 direct interviews with individuals with developmental disabilities. Organize and carryout over 2,500 mail surveys. Oversee data input through specific software and report writing. Collect data from provider agencies. Analyze and present data to funding source, consumers, and national entities. Budget responsibility. Supervise two Project Coordinators, interns, graduate assistants, and office assistant. Oversee and/or conduct approximately 600 annual ICAP assessments statewide. Presentations on research/findings to state and national audiences. 1991-2001 Ark Regional Services Laramie, WYTitle: Family Services CoordinatorDuties: Coordinated agency case management with supervision of five case managers. Ensured compliance with State and Federal Waiver regulations through individual care plans for a combined total of over $4 million annually. Provided oversight to over 100 annual Individual Care Plans. Directed agency intake and eligibility including review of documentation of disability. Supervised all internships and practicum experiences, provided oversight for individual records, acted as Safety Committee Chair, and directed compliance with incident/accident protocol. Mandt instructor. Case manager for 15 individuals. 1990-1991 Ark Regional Services Laramie, WYTitle: Case ManagerDuties: Service coordinator for 25 individuals with developmental disabilities. Coordinated a team process of care plan synthesis ensuring that the individual with disability is represented either in person or by guardian. Ensure that choice of services is provided. Provide information/access to assistive technology, as needed. Coordinate a personal profile for each individual as to the person's individual characteristics, wishes and desires. Ensure timely healthcare appointments. Maintain contact with family/guardians.
