SUMMARY

Seasoned case management professional with extensive experience developing, evaluating, and implementing individualized care and action plans. Broad base of experience serving as an advocate for the welfare of families and individuals acting as a liaison between clients, their families, government agencies, and service providers. Currently providing case management services, that specializes in rehousing, for the population of homeless women, with children that will provide up to three months financial assistance toward locating obtaining and maintaining permanent housing.
