Description

Survey data findings from Southern AZ Person-Centered Planning Model Program participants which include youth with developmental disabilities (age 16 through 22) transitioning to adulthood, family members and DES/DDD support coordinators. Data reveals the impact and effectiveness of person-centered planning from the perspective of all three participant sets.
