Description

A collaboration among Iowa?s UCEDD, Title V CSHCN program, and DHS, Community Circle of Care is a SAMHSA-funded system of care for youth with emotional/behavioral challenges. CCC uses a medical model combined with social supports to deliver family driven services. CCC is working to develop a system of care statewide.
