Experience a hands-on introduction to Taking Charge of My Health: Partners in Health Transition, a new web-based learning site providing information, tools and guidance for teens and young adults with I/DD, family members and caregivers who wish to effectively navigate the health transition process, access health services and ensure a healthy lifestyle. Explore nine self-paced, parallel learning modules with content based on a self-determination and health self-management framework.
